HIPAA Employee Nondisclosure Agreement

This agreement is between an employee and a clinic/agency to prevent the unauthorized disclosure of confidential information by the employee. (0720)

Letter of Termination to Client for Missed Appointments

This letter template can be used by therapists to terminate services with a client who repeatedly misses appointments. (0720)

Letter of Termination to Client Due to Conflict of Interest

This letter template can be used to inform a client the therapist must terminate services when there is a conflict of interest that makes it difficult or impossible for the therapist to treat the client under ethical guidelines. (0720)

Treatment Termination Due to Lack of Progress Letter

Responsibility Release (Client Self-Discharge)

This form can be used when a client chooses to terminate treatment against the advice of the therapist. (0720)

Insurance Payer Appeals Letter (Timely Filing)

This letter can be used by therapists to appeal an insurance payer denial when a claim has been denied due to timely filing. (0720)

Volunteer Confidentiality Agreement

This form can be given to volunteers to ensure they understand the importance of preserving confidentiality when using, discussing, storing, transmitting, or otherwise handling client information. (0720)

Parental Consent for Treatment of Minors

This form can be used when working with children and teens in individual treatment. It is recommended that clinicians meet with the parents/guardians, and then with the children or teens, as appropriate to their developmental level. This will assist discussions with the child regarding confidentiality, information sharing, and records access. (0720)

Telehealth Group Counseling Agreement

This form can be given to clients who are participating in group counseling via a telehealth platform. It reviews the nature of online groups, confidentiality, safety, and the risks and benefits of these services. (0720)
