Scope of Practice Form

This Practice Management Form is designed to define the therapist’s role in providing therapy including: qualifications and credentials, description of practice, boundaries and scope limitations, and professional ethics and conduct. It is intended to be signed by the therapist and the client. (0523, practice management)

Client Commitment to Treatment Form

This form is intended to help understand that therapy can only be successful when they commit to the process. The contract format spells out specific behaviors expected from the client like showing up at sessions on time, trying new behaviors, using a safety or crisis plan when needed, and so on.
